NCAOHN Award Forms
The OHN of the Year/Medique Leadership Award.is presented each year to a member of NCAOHN who has demonstrated excellence in leadership in the field of occupational health nursing. The 2010 recipient will be recognized for their contribution during the NCAOHN Fall Conference, September 29 – October 1, 2010 at the Little River Golf and Resort, Carthage, NC
Nominees will be rated on a specified point evaluation scale based on the award criteria. The attached Criteria, Nomination and Scoring Forms are only sent to NCAOHN officers and chapter presidents. They will also be sent to the NCAOHN website where a list of past winners can be found. Please distribute these forms and share this information with other association members so that these important nominations can be made. Each chapter has an OHN deserving of this honor, so please don't hesitate.
Remember the nominee can complete the nomination form.
